Dedicated to enhancing patient outcomes, the full-time Minnesota Licensed Heart Failure RN will provide bedside care in a hospital setting, manage outpatient clinic visits, and conduct occasional remote work, focusing on education and ongoing disease management for heart failure patients. Key responsibilities Provide bedside care and support for heart failure patients in the hospital Manage new and follow-up outpatient clinic visits for heart failure management Educate patients on disease management and provide ongoing support across the continuum of care Required qualifications Graduate from an accredited nursing program required Current Minnesota RN License required BSN or MSN preferred; BSN or higher required within 36 months of hire Basic Life Support (BLS) certification from the American Heart Association required 1 year of recent cardiology work experience preferred
