Mild/Moderate Itinerant Case Manager

nSchedule: Full-Time, Monday-Friday
nCaseload: Approximately 55-65 Students (All Grade Levels)
nPrograms Served: Mild/Moderate Support Needs and Extensive Support Needs (ESN)
nPosition Overview
nWe are seeking a dedicated and highly organized Mild/Moderate Itinerant Case Manager to support students receiving special education services across multiple grade levels and programs. This position is office-based at one of our school sites and focuses exclusively on special education case management responsibilities.
nThe ideal candidate will have strong knowledge of IEP compliance, special education procedures, and collaborative communication practices. This role does not include instructional or classroom teaching responsibilities.

Prepare, coordinate, and facilitate:

Annual IEP meetings

Triennial IEP meetings

Transition IEPs
n n - n
IEP addendums
n n - n
Ensure compliance with federal, state, and district special education requirements.

Schedule IEP meetings and invite all required participants.
n n
Assessment & Documentation

Conduct formal and informal assessments for IEP development and review purposes.
n n - n
Complete student observations as needed.
n n - n
Develop and maintain accurate special education documentation.
n n - n
Write goal progress reports and updates.
n n - n
Update and affirm student records within the district's special education information system.
n n - n
Complete follow-up actions after IEP meetings with service providers and team members.

Collaboration & Family Communication

Serve as the primary point of contact for parents regarding issues related to IEP implementation and academic supports.

Communicate accommodations and support needs to teachers and site administrators.
n n - n
Facilitate communication among families, teachers, administrators, and related service providers regarding student IEPs.

Attend IEP meetings in person when meetings are held on-site.

What This Position Does NOT Include
nThe Itinerant Case Manager position is strictly focused on case management duties and does not include classroom teaching responsibilities such as:

Curriculum planning and lesson preparation

Direct lesson delivery or instruction

MAP testing administration

Goal instruction or mastery monitoring through classroom teaching

Writing general education report cards
n n - n
Managing classroom supplies or classroom operations

Responding to parent concerns unrelated to IEP implementation

Serving as the teacher of record for instructional services

Qualifications
nRequired

Valid California Education Specialist Credential (Mild/Moderate Support Needs preferred)

Knowledge of special education laws, regulations, and IEP compliance requirements
n n - n
Experience facilitating IEP meetings and managing special education caseloads
n n - n
Strong written, organizational, and interpersonal communication skills

Preferred

Experience working with both Mild/Moderate and Extensive Support Needs (ESN) programs

Experience conducting educational assessments and observations

Familiarity with SEIS or other special education data management systems

Previous case management experience in a K-12 educational setting

nMinimum Requirements:

Bachelor’s Degree from an accredited university preferred

Valid state teaching certificate as required by state, contract/district regulations

Minimum of one year experience in teaching environment preferred

State Teacher Certification; Type: Standard Elementary Teaching preferred

State Teacher Certification; Type: Standard Secondary Teaching preferred

State Teacher Certification; Type: Standard Special Teaching preferred
n n - n
Endorsement: Learning Behavior Specialist (LBS1) preferred

Complies with all relevant professional standards of practice

Current CPR if applicable
n n - n
TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Must be at least 18 years of age
