Providing ongoing remote care management, the full-time Michigan Licensed Nurse Care Manager will monitor patient vitals, develop individualized care plans, and deliver education and coaching to improve patient outcomes while collaborating with physicians and cross-functional teams. Key Responsibilities Monitor and interpret patient vitals, assessments, and alerts within the care management platform Develop, implement, and adjust individualized care plans addressing clinical and social needs Deliver patient education and coaching via telephone and secure messaging to empower chronic condition management Required Qualifications Bachelor's degree in nursing or a closely related healthcare field Active and valid registered nurse (RN) license in good standing in the state of Michigan Verbal and written proficiency in Spanish Approximately two (2) to five (5) years of direct patient care experience in medical-surgical or home care Ability to work independently in a remote, technology-enabled environment
