To support the expansion of the Collaborative Care integrated behavioral health program, the full-time Michigan Licensed Behavioral Health Clinician will provide high-quality patient support through individual assessments, treatment plan development, and psychotherapy, while working remotely to coordinate care for patients with behavioral health needs and chronic health conditions. Key responsibilities Screen and assess patients for mental health and substance abuse disorders, facilitating treatment plans and follow-up care Provide brief behavioral interventions using evidence-based techniques and support psychotropic medication management Participate in weekly caseload consultations with the psychiatric consultant and document patient progress in the EHR Required qualifications A minimum of a master's degree in social work, psychology, counseling, rehabilitation, or a related field 3-5+ years of experience in behavioral health intervention clinical work, case management, or community care coordination Active independent clinical license in Michigan (LMSW, LPC, LMFT, or Licensed Psychologist) Current enrollment in the state's Medicaid & Medicare is preferred Proficiency in utilizing electronic health records (EHR) and remote communication systems
