Transitions Care seeks a Medical Director to provide clinical leadership for our hospice program, ensuring high-quality, compassionate care for patients with serious or terminal illness. The Medical Director oversees symptom management, certifies and recertifies eligibility, and partners with an interdisciplinary team to develop and review individualized care plans.
