- Leadership experience in NICU or clinical program development - REQUIRED
- Neonatology experience - REQUIRED (experienced candidates only)
- Does the provider require visa support (J1/H1B)? - REQUIRED to specify
- Most recent NPDB Self Query Report (within 30 days active) - REQUIRED no exemption, required at presentation
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - REQUIRED no exemptions
- MD or DO degree
- Eligible for hospital credentialing
- Leadership experience in clinical quality or program development (implied requirement)
