About This Role

As a PACE Medical Director at myPlace Health, you will oversee day‑to‑day clinical operations at assigned center(s) and serve as the accountable medical leader, guiding and supporting your clinical team to achieve exceptional outcomes. You will shape the future of our program through quality improvement initiatives and champion strong clinical results while ensuring a seamless experience for each participant and their caregivers.

What Does Success Look Like?

Clinical Care and Oversight

  • Champion exceptional, evidence‑based, and cost‑effective medical services for PACE participants, aligned with clinical standards and regulatory guidelines.
  • Provide comprehensive medical care – evaluations, diagnoses, treatment plans, and coordination with specialists.
  • Maintain meticulous, compliant documentation of every participant’s care story.
  • Recruit, train, supervise, and mentor physicians, advanced practitioners, and care coordinators to foster a culture of excellence.
  • Train and oversee contracted primary care physicians, advanced practitioners, and specialists to ensure high standards across all contacts.

Quality and Utilization Management

  • Lead the Quality Improvement and Performance Improvement (QAPI) Program.
  • Monitor utilization trends (e.g., hospitalizations, ER visits) and set outcome targets to guide improvement efforts.
  • Design interventions that enhance participant health, quality of life, and satisfaction.
  • Oversee chart audits for quality, compliance, medication practices, and risk areas.
  • Ensure robust primary‑care staffing and manage after‑hours on‑call arrangements for 24/7 availability.

Team Leadership and Collaboration

  • Provide consultation on grievances, appeals, and IDT decision‑making.
  • Participate in the Quality Committee and other workgroups to drive organizational improvement.
  • Build and nurture relationships with community physicians and providers to facilitate referrals.
  • Promote teamwork, accountability, and professional development through meetings and coaching.

Ideal Candidate Profile

  • MD or DO with active board certification in geriatrics, complex care, home‑based care, palliative care, internal medicine, family medicine, or a related area (maintained throughout employment).
  • At least five years of medical practice, including two years working directly with elderly or frail populations.
  • Licensed in all states where we operate, with a valid DEA certificate and up‑to‑date immunization records.
  • Experienced in PACE or similar integrated care models.
  • Strong passion for serving high‑risk seniors and frail older adults.
  • Exceptional communication, leadership, and collaborative mindset.
  • Bilingual in English and Spanish preferred.
