The Optumcare Medical Behavioral Integration Nurse Case Manager (MBI NCM) serves as a direct clinical liaison between OptumCare and the clinical network, focusing on assessing, planning, implementing, coordinating, monitoring, and evaluating services to meet members' health needs.

What You'll Do
- Screen and identify members with high-risk, long-term chronic conditions for care management services.
- Perform member evaluations and onsite visits in various settings.
- Conduct comprehensive assessments for members with high-risk symptoms and multiple co-morbidities.
- Document findings and develop individualized care plans compliant with CMS regulations.
- Utilize advanced clinical skills to meet members' health, behavioral health, and psychosocial needs.
- Document patient/family status, diagnosis, treatment plans, and progress in electronic medical records.
- Advocate for members and families to ensure their needs are represented by the healthcare team.
- Attend interdisciplinary team meetings regarding patient care and collaborate with providers.
- Facilitate MD consultations on complex cases using professional knowledge and critical thinking.
- Manage assigned case load efficiently and effectively.
Requirements
- Education: Active and unrestricted RN license required
- Experience: 2+ years of experience in nursing, preferably in a related field-based case management
- Licenses & Certifications: RN (Registered Nurse) - State Licensure required
- Driver's License and access to reliable transportation
