MDS Coordinator (RN)
Daytona Beach Health and Rehabilitation Center is seeking an experienced and detail-oriented Registered Nurse to join our team as an MDS Coordinator .
This position is ideal for an RN interested in moving into a primarily administrative nursing role while continuing to use strong clinical assessment, documentation, and critical-thinking skills. The MDS Coordinator works closely with an interdisciplinary team to ensure resident assessments, care plans, and supporting documentation are accurate, complete, and compliant with applicable requirements.
Position Overview
Under the direction of the Director of Nursing, the MDS Coordinator leads the interdisciplinary resident-assessment process. This role uses clinical and functional information to support comprehensive assessments and individualized care plans centered on each resident’s needs, preferences, and goals.
The ideal candidate will have excellent documentation and assessment skills, strong attention to detail, and the ability to effectively coordinate with nursing, therapy, dietary, social services, and other members of the care team.
Key Responsibilities
- Coordinate the interdisciplinary assessment process for assigned residents
- Complete and coordinate MDS assessments accurately and within required timeframes
- Coordinate the development and completion of comprehensive resident care plans
- Ensure care plans comply with facility policies and applicable state and federal requirements
- Monitor resident documentation to ensure care plans remain current and reflect each resident’s needs
- Identify documentation gaps or inconsistencies and communicate concerns to the Director of Nursing and interdisciplinary care team
- Participate in care-plan meetings and support communication with residents and families
- Educate team members on assessment, documentation, and care-planning requirements
- Maintain accurate electronic documentation and supporting clinical records

Qualifications
- Active Registered Nurse license in the State of Florida in good standing
- Knowledge of professional nursing standards, clinical practices, and resident care
- Strong clinical assessment and documentation skills
- Excellent analytical, organizational, and critical-thinking abilities
- Strong attention to detail and ability to manage deadlines
- Computer proficiency and experience with electronic clinical documentation
- MDS, long-term care, skilled nursing, reimbursement, or care-planning experience preferred
