Key Responsibilities:

  • Complete and submit accurate and timely MDS assessments per federal and state regulations.

  • Coordinate interdisciplinary team meetings to develop individualized resident care plans.

  • Monitor and track assessment schedules to ensure compliance with regulatory timelines.

  • Analyze MDS data to support quality improvement initiatives and reimbursement optimization.

  • Educate staff on care documentation standards and regulatory changes.

  • Collaborate with nursing, therapy, and administrative teams to ensure resident-centered care.

  • Participate in audits, surveys, and inspections as needed.

Qualifications:

  • Current RN or LPN/RN license in MA

  • Minimum of (1-2) years of experience in long-term care or skilled nursing facility.

  • Prior MDS experience is preferred (MDS 3.0 knowledge is highly desirable).

  • Certification in Resident Assessment Coordination (RAC-CT) preferred or willingness to obtain.

  • Strong understanding of Medicare and Medicaid reimbursement processes.

  • Exceptional attention to detail, organization, and communication skills.

  • Ability to work collaboratively in a team environment.
