Responsibilities
- MDS Director: Oversee the entire MDS process, ensuring compliance with federal and state regulations, and accrediting agency standards.
- Develop and implement policies, procedures, and protocols to ensure accurate and timely completion of MDS assessments, care plans, and care delivery.
- MDS Assessment: Conduct thorough assessments of patients' physical, psychological, and social needs, identifying opportunities for improvement in care delivery.
- Care Planning: Collaborate with the interdisciplinary team to develop individualized care plans that meet patients' specific needs and goals.
- Care Delivery: Ensure patients receive high-quality, person-centered care that addresses their physical, emotional, and social needs.
- Directly supervise and mentor MDS coordinators, therapists, and other healthcare professionals to ensure exceptional patient care.
Requirements
- Bachelor's Degree: In Nursing, Healthcare Administration, or a related field (or equivalent experience).
- Minimum 2+ Years of Experience: In a leadership role, preferably in a Skilled Nursing Facility.
- Strong MDS Knowledge: Demonstrate expertise in MDS assessment and care planning principles, policies, and regulations.
- Excellent Communication: Ability to effectively communicate with patients, families, and healthcare professionals.
- Strong Leadership: Proven leadership and management skills, with the ability to inspire and motivate team members.
