Apply today.Your Role as the MDS Coordinator Americare Senior Living is seeking a detail-oriented and clinically skilled MDS Coordinator to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community.
Summary The MDS Coordinator is responsible for coordinating and completing the MDS process as required by federal and state regulations.
This role ensures accurate, timely, and compliant clinical documentation, supports care planning and Medicare processes, and collaborates with nursing leadership, the Administrator, and the interdisciplinary team.
ResponsibilitiesCoordinate and complete MDS assessments in accordance with federal and state guidelinesEnsure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and currentComplete assigned MDS sections, assessments, and RAP summariesReview, update, and maintain resident care plans based on assessment findingsParticipate in Medicare meetings, care plan meetings, and interdisciplinary team discussionsAssist in maintaining compliance with state, federal, and Americare standardsSupport survey readiness and participate in inspections as neededCommunicate changes in resident condition to the Administrator and nursing leadershipAssess nursing supply and equipment needsProvide on-call support to staff as neededQualificationsGraduate of an accredited school of nursingCurrent Registered Nurse (RN) license requiredExperience with MDS 3.0 and skilled nursingStrong attention to detail, organization, and analytical skillsAbility to collaborate effectively with interdisciplinary teamsCompany Information Americare Senior Living is committed to equal employment opportunities and welcomes applicants from all walks of life.
