MDS Coordinator (LPN)
Agawam West Rehab & Nursing is seeking an experienced LPN for a full-time, salaried MDS Coordinator position.
The MDS Coordinator will oversee the coordination and completion of resident assessments, ensuring accurate clinical documentation, timely MDS completion, and appropriate reimbursement. This role works closely with nursing, therapy, and the interdisciplinary team to maintain accurate and comprehensive resident assessments.
Responsibilities
- Coordinate, schedule, and complete MDS assessments
- Collect and review clinical information from nursing, therapy, and medical records
- Ensure accurate and timely documentation and submissions
- Participate in interdisciplinary care planning and resident assessment reviews
- Support regulatory compliance and accurate reimbursement
- Maintain knowledge of current MDS and CMS requirements
Qualifications
- Active Massachusetts LPN license required
- MDS experience in a skilled nursing facility required
- Strong understanding of the MDS process, including coordination, scheduling, and completion
- Long-term care or rehabilitation experience
- Strong organizational and attention-to-detail skills
Compensation & Benefits
- Salary: $91,000 - $95,000 per year (DOE)
- Quality Measure Bonus: Up to $7,500 per year
