Salary: $100,000–$130,000 per year
Palm Gardens Center for Nursing & Rehabilitation is seeking an experienced MDS Coordinator (RN) to join our team.
We are looking for a knowledgeable, organized, and motivated MDS professional who understands the importance of accurate assessments, strong clinical documentation, compliance, and reimbursement.
Responsibilities
Coordinate and complete MDS assessments accurately and timely
Manage the MDS/RAI process and assessment schedule
Ensure compliance with CMS, Medicare, Medicaid, and regulatory requirements
Review clinical documentation for accuracy and completeness
Work closely with nursing, therapy, social services, and the interdisciplinary team
Monitor PDPM documentation and reimbursement
Identify documentation opportunities and ensure appropriate follow-up
Participate in care planning and interdisciplinary meetings
Qualifications
Active RN license
MDS experience in a skilled nursing facility required
Strong knowledge of MDS 3.0, RAI guidelines, PDPM, and Medicare
Strong organizational and communication skills
Excellent attention to detail
