Maternity Care Coordinator

Serving as the single point of contact for the Mom and Baby Beginnings program, the full-time remote Maternity Care Coordinator will engage and enroll members, conduct health assessments, and coordinate care services while providing quality customer service and maintaining accurate documentation. Key responsibilities: Telephonically engage and enroll members in the Mom and Baby Beginnings program Conduct initial screenings and needs assessments, considering cultural and linguistic needs Coordinate care and collaborate with internal teams and healthcare providers to develop Person-Centered plans Required qualifications: Diploma or Associates in a healthcare-related field or equivalent work experience required; Bachelor's degree preferred A minimum of three years of experience in healthcare, including case management or care coordination Intermediate proficiency with Microsoft Office applications Knowledge of healthcare laws and case management practices Strong understanding of cultural and demographic diversity
