As our CC Master, Social Worker, you will plan, implement, and organize social work resources, and evaluate and provide treatment regarding mental health issues for patients, facilitating their linkage to community resources.
Every day you will assess patient needs, deliver mental health treatment as appropriate, develop comprehensive care plans, and ensure seamless connections to relevant community support systems, all in coordination with continuing care.
To be successful in this role, you will possess an MSW, clinical expertise in mental health treatment, strong resource navigation skills, and the ability to effectively plan, implement, and evaluate social work interventions to support continuing patient care and community integration.
- Develop and implement patient's plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team.
- Assess patient's physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.
- Evaluate patient's response and intervene to ensure optimal patient outcome.
- Promote, advocate, and collaborate to protect the health, safety, and rights of each patient.
- Demonstrate competency in a variety of therapeutic/diagnostic interventions.
- Use quality measures to improve performance and accountability for patient outcomes, patient experiences, and safe delivery of care.
Required:
- Master's Degree in Social Work
- LSW required
- Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED
- Valid Driver's License
Preferred:
- Licensed Clinical Social Worker (LCSW)
