In this role, you will complete psychosocial assessments, develop and update care plans, provide counseling, crisis intervention, and resource navigation, and advocate for patients within an interdisciplinary clinical team. You will coordinate with nurses, physicians, and dietitians to address social, emotional, and financial barriers to treatment adherence, document in compliance with regulatory standards, and support patient education, adjustment to dialysis, and transitions of care in a mission-driven environment.
