Key responsibilities Lead internal and joint case rounds to ensure effective care plan development and execution Coordinate with network and longitudinal care providers for optimal service delivery Monitor and analyze network data to identify trends and implement necessary interventions Required qualifications Master of Science in Nursing (MSN) or Master of Science in Physician Assistant studies Five years of clinical experience in palliative care, hospice, or working with patients with serious illness Active Registered Nurse or Physician Assistant license in Massachusetts without any board actions Board certification as an advanced practice nurse by a nationally recognized certifying agency Active Unrestricted DEA license
