Working remotely during Eastern Standard Hours, the full-time Massachusetts Licensed Pharmacist will provide expertise in Medication Therapy Management (MTM) and clinical pharmacy services, focusing on optimizing medication-related health care outcomes and supporting quality cost-effective member care. Key responsibilities Facilitates MTM support through comprehensive medication reviews and communication with members and providers Analyzes medication profiles to identify potential safety issues and makes evidence-based recommendations for optimization Collaborates with interdisciplinary teams to coordinate care and follows up on members' progress toward drug treatment goals Required qualifications Doctor of Pharmacy (PharmD) with an active and unrestricted Massachusetts pharmacist license At least 3 years of experience in a pharmacy, MTM, PBM, or managed care setting Demonstrated knowledge of clinical pharmacology and disease management Clinical skills related to Medication Therapy Management (MTM) Proficiency in Microsoft Office suite and data reporting
