Massachusetts Licensed Ambulatory Care Pharmacist

To support a growing healthcare team, the full-time Massachusetts Licensed Ambulatory Care Pharmacist will provide Comprehensive Medication Management (CMM) remotely, collaborating with primary care providers and specialists to promote safe and effective drug therapy while managing chronic disease states. Key responsibilities: Provide Comprehensive Medication Management (CMM) in collaboration with clinical care teams Develop medication management action plans to achieve member self-management goals and clinical outcomes Communicate and collaborate with care team members to ensure alignment with medication-related goals Required qualifications: Active, unrestricted Massachusetts pharmacist license Completion of a PGY-1 Pharmacy Practice Residency and a PGY-2 Ambulatory Care Pharmacy Residency, or At least 2 years of clinical pharmacist experience in an ambulatory care setting
