Seeking a Maryland Licensed Nurse Practitioner with cardiology experience, the full-time position will manage patients in an outpatient virtual transition of care heart failure clinic, focusing on comprehensive evaluations, guideline-directed medical therapy, and collaboration with an interdisciplinary team. Key Responsibilities Virtually perform comprehensive assessments and manage heart failure patients according to clinical guidelines Collaborate with cardiologists and primary care providers to ensure seamless transitions of care and timely follow-ups Provide education to patients and families on heart failure management and medication adherence Required Qualifications 1 year of cardiology experience, preferably in heart failure Acute, family, or primary care certification BLS certification Ability to work collaboratively in an interdisciplinary team Experience with virtual patient management and care coordination
