Maine Licensed Social Worker

To support Maine residents with disabilities facing complex healthcare and service coordination challenges, a hybrid Licensed Social Worker position will provide case coordination, community collaboration, and advocacy to ensure access to appropriate services and safe living environments. Key Responsibilities Provide consultation and support to clients and care teams addressing complex healthcare, financial, and housing issues Facilitate multidisciplinary team meetings to develop safe discharge plans and identify service barriers Maintain accurate documentation and track case activity in compliance with departmental protocols Required Qualifications Current Licensed Social Worker (LSW) credential issued by the Maine State Board of Social Worker Licensure Six years of education and/or professional experience in social services casework or related fields Experience in case management or care coordination Knowledge of healthcare systems and community-based service delivery models Understanding of client rights and ethical practice standards
