Supporting high-acuity member populations, the full-time Maine Licensed Population Health Coordinator will manage data and registry tasks, coordinate care for complex cases, and provide direct member care, all while pioneering innovative care delivery methods in a collaborative environment. Key responsibilities Maintain and assess an accurate registry of members, facilitating reviews and proposing quality improvement initiatives Perform internal and external care coordination tasks for complex members and assist in training staff on the Collaborative Care Model Provide direct support to assigned members through telephonic outreach, screenings, and educational interventions Required qualifications Registered Nurse (RN) or Maine Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license Bachelor's degree in Nursing preferred At least 5 years of direct patient-care experience in addiction medicine or related behavioral health fields Strong understanding of care coordination and case management Proficiency in multiple EMRs and Microsoft Office/Google Suite, with spreadsheet proficiency required
