Providing leadership in patient-centered care coordination, the full-time Maine Licensed Case Manager RN will develop safe discharge plans and collaborate with multidisciplinary teams to ensure optimal transitions in care, all while working remotely. Key Responsibilities Coordinates patient care processes to achieve desired quality outcomes and promotes continuity of care Facilitates communication among healthcare team members and participates in multi-disciplinary team care conferences Integrates assessment of post-hospital service needs and educates patients and families on discharge plans Required Qualifications Associate's degree in Nursing Current, unrestricted compact RN license in the state of residence 3+ years of experience in a hospital, acute care, or direct care setting Intermediate level of proficiency in typing and navigating a Windows-based environment
