To support the Enrollments team, the full-time California Licensed Vocational Nurse will engage with prospective clients through 60-100 outbound calls daily, educate them on Enhanced Care Management services, and facilitate their enrollment while working remotely. Key responsibilities Engage with prospective clients and document interactions in the Client Relationship Management (CRM) system Assist clients in completing psychosocial assessments and developing personalized care plans Review medical charts to understand client needs and recommend appropriate interventions Required qualifications Active California Licensed Vocational Nurse (LVN) license, Medical Assistant certificate, or Bachelor's degree in a related field Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment Experience working with complex care clients and ECM Populations of Focus Familiarity with technology, including video conferencing and electronic health records Bilingual (English/Spanish) is highly preferred
