To facilitate seamless patient transitions through the Reserved Bed Program, the full-time Reserved Bed Post-Acute Care Manager will serve as the primary liaison between hospital partners and post-acute care centers, coordinating referrals, managing patient placements, and maintaining program metrics while working remotely. Key responsibilities Educate hospital case managers on the Reserved Bed Program and manage the referral process to ensure timely patient placements Coordinate patient transfers to PACS facilities by arranging necessary documentation and services prior to admission Track and report key performance indicators related to referral activity, admissions, and program outcomes to ensure compliance and program growth Required qualifications LVN or RN license required High school diploma or GED; Bachelor's degree in health care or related field preferred Two (2) years of Admissions experience preferred Proficiency in Microsoft Suite products and hospital EMAR systems Ability to maintain a professional and distraction-free remote work environment
