LVN / LPN LTC/SNF

Long-Term Care LPN Opportunity

Seeking an experienced Long-Term Care LPN for a traveler assignment to provide compassionate nursing care within a skilled nursing facility. The nurse will deliver high-quality support to residents, ensuring their medical needs are met accurately and efficiently.

Responsibilities:

• Administer medications and treatments in accordance with care plans.
• Conduct thorough assessments of residents' health status and document findings accurately.
• Develop and update individualized care plans in collaboration with interdisciplinary teams.
• Coordinate effectively with physicians, care teams, and residents' families.
• Maintain compliance with healthcare regulations and facility policies.

Qualifications:

• Hold an active Hawai’i LPN license.
• Possess a current BLS certification.
• Demonstrate a minimum of one year of recent Long-Term Care or skilled nursing experience.
• Have experience with electronic medical records systems.
• Show strong clinical assessment and communication skills.
