Job Summary:
Provide high-quality nursing care to residents in a skilled nursing or long-term care facility. Support patient well-being through medication administration, patient assessments, and collaboration with the healthcare team.
Responsibilities:
- Administer medications and treatments accurately according to physician orders.
- Conduct patient assessments to monitor health status and report changes.
- Maintain detailed and compliant documentation of care provided.
- Collaborate with registered nurses and other healthcare professionals to develop care plans.
- Ensure adherence to safety protocols and infection control standards.
- Educate patients and their families on health conditions and care plans.
- Valid LVN or LPN license in the state of practice.
- Proven experience working in LTC or SNF settings preferred.
- Ability to perform patient assessments and administer medications competently.
- Excellent communication and teamwork skills.
- Demonstrate attention to detail and adherence to regulatory standards.
- BLS certification required; additional CNA or geriatric care experience is a plus.
