LVN / LPN LTC/SNF

n Job Summary:n
n Provide skilled nursing care to residents in a Long-Term Care/SNF facility as an LVN/LPN. Deliver compassionate patient care while supporting the healthcare team and maintaining compliance with facility and regulatory standards.n

n Responsibilities:n

• n Administer medication and treatments accurately according to physician orders.n
• n Monitor and document patient vital signs, symptoms, and progress.n
• n Assist with patient admissions, discharges, and care planning.n
• n Collaborate with interdisciplinary team to ensure high-quality resident care.n
• n Maintain healthcare records with precision and confidentiality.n
• n Follow infection control procedures and safety protocols diligently.n

n Qualifications:n

• n Valid LVN or LPN license in the state of practice.n
• n Minimum of 1 year of experience in a LTC or SNF setting preferred.n
• n Strong knowledge of medical terminology, nursing practices, and safety standards.n
• n Demonstrate excellent communication and interpersonal skills.n
• n Ability to work effectively in a team environment and handle multiple priorities.n
