LVN / LPN LTC/SNF

Long-Term Care Nursing Position

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure resident safety, comfort, and well-being through comprehensive nursing procedures and compassionate care.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status and report changes to the medical team.
• Assist residents with activities of daily living, including hygiene, mobility, and dietary needs.
• Follow facility protocols and maintain compliance with state and federal regulations.
• Ensure accurate documentation of care provided and maintain confidentiality.
• Collaborate with interdisciplinary team to develop and implement care plans.

Qualifications

• Hold a valid LVN or LPN license in the applicable state.
• Possess current CPR and Basic Life Support (BLS) certifications.
• Demonstrate excellent communication and clinical skills.
• Have previous experience in LTC or SNF settings preferred.
• Ability to work independently and as part of a team in a fast-paced environment.
• Be able to bring a valid state or federal-issued ID and wear a photo badge during shifts.
