Job Summary:

Responsibilities:
- Administer medications and treatments following physician orders and facility protocols
- Monitor patient vital signs and record observations accurately
- Assist with activities of daily living and basic patient hygiene
- Collaborate with interdisciplinary team to develop and update care plans
- Maintain comprehensive documentation of patient care activities
- Ensure infection control procedures and safety standards are consistently upheld
- Valid and current LVN or LPN license issued by the state of New Hampshire
- Proven experience in long-term care or skilled nursing facilities
- Ability to accurately assess patient conditions and respond appropriately
- Strong interpersonal and communication skills
- Knowledge of state and federal healthcare regulations and compliance standards
