The LVN Home Health professional provides essential nursing care to patients in a home health setting, ensuring their comfort, safety, and well-being. This role involves administering medications, monitoring vital signs, managing wound care, educating patients and families, and maintaining accurate documentation while effectively communicating with the healthcare team.
Responsibilities
- Provide direct patient care and support in the home
- Manage and administer medications as prescribed
- Perform wound dressing and monitor wound healing progress
- Monitor and record vital signs regularly
- Educate patients and families about care plans and health management
- Document patient information and clinical data accurately
- Communicate effectively with patients, families, and healthcare team members
Preferred Qualifications
- At least 1 year experience in licensed vocational nursing
- Skilled in wound care and medication administration
- Proficient in monitoring vital signs and infection control practices
- Familiarity with electronic health records (EHR)
- Strong communication, empathy, and time management skills
