Responsibilities
- Conduct comprehensive assessments of patients' physical, emotional, and cognitive needs.
- Develop individualized treatment plans based on patient assessments and goals.
- Provide evidence-based therapeutic interventions to LTC patients in an outpatient setting.
- Monitor patient progress and adjust treatment plans as necessary.
- Collaborate with interdisciplinary teams including physicians, nurses, social workers, and family members to ensure coordinated care.
- Document patient sessions, progress notes, and treatment plans accurately and timely in compliance with regulatory standards.
- Educate patients and their families about treatment goals, techniques, and strategies for managing conditions.
- Maintain patient confidentiality and adhere to ethical and professional standards.
- Participate in continuing education and professional development activities.
- Master's degree in Physical Therapy, Occupational Therapy, Speech Therapy, Psychology, Social Work, or related field.
- Valid and current licensure or certification as required by state regulations.
- Experience working with long-term care patients in an outpatient or similar setting preferred.
- Strong knowledge of therapeutic techniques and interventions relevant to LTC populations.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in electronic health records (EHR) and documentation standards.
- Compassionate, patient-centered approach to care.
