Supporting NorthStar Members in navigating their healthcare systems, the full-time LTC Care Coordinator will manage a caseload, perform care management services, and ensure effective communication within an interdisciplinary care team while working remotely. Key responsibilities: Receive and document incoming calls from members regarding benefits, care coordination, and program inquiries Manage an assigned member caseload requiring short-term care coordination and health navigation Perform outreach to confirm services are in place and monitor member screening effectively Required qualifications: Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training/experience High school diploma or GED Experience in a fast-paced environment managing multiple priorities Effective verbal and written communication skills with attention to grammar and punctuation
