Compensation

$61,547.20 annual salary + $5,000 hiring bonus — total compensation package valued at $66,547.20.

Reports to: MyCare Ohio Case Manager Supervisor

FLSA Status: Exempt

Position Summary

As an LSW Case Manager, you will support long-term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs through clinical assessment, care coordination, advocacy, and ongoing case management. This opportunity combines healthcare leadership, community impact, and hybrid field-based flexibility while improving quality outcomes for older adults and individuals with disabilities.

  • Conducting comprehensive client assessments
  • Developing and implementing individualized care plans
  • Coordinating home and community-based services
  • Advocating for client needs and quality care outcomes
  • Monitoring service effectiveness and client well-being
  • Collaborating with interdisciplinary teams, providers, families, and community partners

Qualifications
  • Current licensure as a Licensed Social Worker (LSW) through the State of Ohio Counselor and Social Work Board
  • Minimum of one (1) year of experience working with individuals with physical or behavioral disabilities in home healthcare, medical social work, or behavioral health environments
  • Excellent communication, interviewing, and assessment skills
  • Knowledge of community resources available to older adults and individuals with disabilities
