Supporting multiple high-volume outpatient gastroenterology clinics, the full-time South Carolina Licensed LPN Care Coordinator will manage Epic In-Basket tasks, triage GI symptom calls, and coordinate patient follow-ups to ensure high-quality care. Key responsibilities Triage GI symptom calls and respond to patient inquiries regarding medications and advice Process medication refill requests and coordinate prior authorizations for medications Assist RNs with care coordination and streamline transitions of care after testing and hospitalizations Required qualifications Current South Carolina LPN License or compact state license Graduate from an accredited Practical Nurse program with at least six months of patient care experience Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification Ability to demonstrate commitment to patient-centered care and effective communication skills Computer literacy and proficiency in managing electronic health records
