Responsibilities:

• Provide direct patient care including monitoring vital signs, administering medications, and wound care
• Document patient information, observations, and care activities accurately in medical records
• Communicate effectively with patients, families, and interdisciplinary care teams
• Perform routine clinical procedures and respond promptly to patient needs
• Educate patients and caregivers on health management, medications, and discharge instructions
• Maintain a safe, clean, and compliant care environment in accordance with regulatory standards

Requirements:

• Active LPN license in the state of New Jersey (required)
• Graduate of an accredited Licensed Practical Nursing program
• Current BLS/CPR certification
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills with patients and staff
• Ability to work independently and collaboratively in a fast-paced environment
• Prior clinical experience in a similar healthcare setting preferred
