Join us and build a rewarding career where you can make a meaningful difference in the lives of others every day.RESPONSIBILITIES:Assist with the oversight of resident care, assessments, care planning, and nursing services on the assigned unitSupport and supervise nursing staff, nursing assistants, and unit personnel under the direction of the Unit ManagerMonitor resident conditions and ensure providers are notified of changes, abnormal results, and care needsEnsure physician orders, medications, treatments, laboratory work, and documentation are completed accurately and timelyConduct rounds, review reports, address resident needs, and communicate pertinent information during shift changesParticipate in care plan meetings, family meetings, discharge planning, and interdisciplinary team meetingsComplete quality assurance audits and support quality improvement initiativesCoordinate appointments, transportation, referrals, and continuity of care across disciplinesAssist with staff education, orientation, policy implementation, and performance managementMaintain unit supplies, address environmental concerns, track infection control and immunization records, and ensure regulatory complianceRespond appropriately to medical, behavioral, and emergency situationsPerform additional duties as assigned by the Unit Manager or SupervisorREQUIREMENTS:Minimum education requirement: High School Diploma or equivalent; Associate's Degree preferredMinimum experience requirement: 1 year of experience as an LPN in a long-term care setting; 3–5 years of LPN experience in long-term care with supervisory experience preferredValid NYS Licensed Practical Nurse (LPN) License requiredStrong clinical assessment, documentation, and care planning skillsAbility to supervise, coach, and support nursing staffExcellent communication, leadership, and customer service skillsStrong organizational, problem-solving, and critical thinking abilitiesKnowledge of long-term care regulations, infection control practices, and quality assurance processesAbility to collaborate effectively with residents, families, providers, and interdisciplinary teamsBENEFITS:Paid Time Off (PTO) for Full-Time and Part-TimeWeekly or Same-Day Pay OptionsHealth, Dental, and Vision InsuranceLife InsuranceEmployee Referral Bonus ProgramRetirement Saving OptionsEqual Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
