Seeking a dedicated Licensed Practical Nurse (LPN) to support individuals with developmental disabilities living in our OPWDD residential settings in Brooklyn. This role is ideal for a compassionate nurse who enjoys hands-on care, building relationships with residents, and working as part of a collaborative interdisciplinary team.
Key Responsibilities
- Provide nursing care to individuals residing in OPWDD residential homes
- Perform phlebotomy/blood draws and coordinate lab work as needed
- Administer medications and treatments in accordance with physician orders and agency protocols
- Monitor residents’ health status and report changes to the RN or medical team
- Maintain accurate documentation and ensure compliance with OPWDD and regulatory standards
- Collaborate with residential staff to support each resident’s individualized health care plan
- Provide health education and guidance to direct support staff
Qualifications
- Active NYS Licensed Practical Nurse (LPN) license
- Proven blood draw / phlebotomy experience (required)
- Experience working in OPWDD, developmental disabilities, or residential settings strongly preferred
- Strong communication and documentation skills
- Compassionate, reliable, and team-oriented
