Licensed Practical Nurse (LPN) – OPWDD Residential Program

Location: Brooklyn, NY

Schedule: Monday to Friday 9a-5p

Salary: $70,000 to $71,253/year

Seeking a dedicated Licensed Practical Nurse (LPN) to support individuals with developmental disabilities living in our OPWDD residential settings in Brooklyn. This role is ideal for a compassionate nurse who enjoys hands-on care, building relationships with residents, and working as part of a collaborative interdisciplinary team.

Key Responsibilities
  • Provide nursing care to individuals residing in OPWDD residential homes
  • Perform phlebotomy/blood draws and coordinate lab work as needed
  • Administer medications and treatments in accordance with physician orders and agency protocols
  • Monitor residents’ health status and report changes to the RN or medical team
  • Maintain accurate documentation and ensure compliance with OPWDD and regulatory standards
  • Collaborate with residential staff to support each resident’s individualized health care plan
  • Provide health education and guidance to direct support staff
Qualifications
  • Active NYS Licensed Practical Nurse (LPN) license
  • Proven blood draw / phlebotomy experience (required)
  • Experience working in OPWDD, developmental disabilities, or residential settings strongly preferred
  • Strong communication and documentation skills
  • Compassionate, reliable, and team-oriented
