LPN Neuromuscular Program Coordinator -Neurology - Full-TimeThe Neuromuscular Program Coordinator at UT Erlanger Neurology will carry out responsibilities which ensure high quality, well-coordinated patient care. Administer vaccinations and monitor for adverse effects. Perform phlebotomy procedures and handle outside referral labs. Administer and coordinate care of medical equipment including feeding tubes, DME, and respiratory equipment. Order of labs, radiological studies, and referrals as dictated by ordering providers.
The Coordinator is responsible for planning, coordinating, and evaluating services provided in the following areas; ALS Clinic, EDX Lab schedule, Neuromuscular physician schedule. The Coordinator is responsible for customer service initiatives, insurance pre-certifications, maintenance of EPIC schedule, quality metrics, coordination of care, including facilitation of diagnostic and therapeutic modalities. The Coordinator will work closely with the Regional Manager, Clinic Medical Director, and Neuromuscular Program Director to ensure that supplies, documentation, billing, scheduling, treatment modalities, referrals, and other follow up activities related to the care of patients presenting to this program. The Coordinator is responsible for developing professional and community relationships that will enable the provision of care and support to those living with Neuromuscular Disease, such as ALS and their families. The Coordinator must have highly developed verbal and written communication skills and the ability to work effectively with administrative, clinical and support staff to accomplish the goals of the program.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices patient population. Participates in the practice's quality improvement process and performance evaluation.Education: Required: Graduate from an accredited School of Nursing.
Preferred: Experience: Required: Two (2) years of hospital or physician practice experience. Preferred: Experience with billing, insurance pre-certifications, clinic scheduling and other aspects related to patient care coordination Position Requirement(s): License/Certification/Registration Required: Current Licensure to practice practical nursing in the State of Tennessee. BLS completed prior to going to the unit/department and maintained going forward
