Licensed Practical Nurse (LPN) - Be the Heart of Exceptional Care
Location: Rockbridge Oaks Senior Living
Position Type: Part time, Full time

What You'll Do - Your Purpose in Action
• Champion resident wellness by identifying and responding to evolving care needs, ensuring residents receive the highest quality of personalized service.
• Conduct timely assessments at move-in, 30-day, semi-annual, and status-change intervals in partnership with the Resident Care Director (RCD).
• Lead monthly wellness visits and monitor clinical status, vitals, weights, and medication profiles with care and accuracy.
• Respond to incidents such as falls or changes in condition with proper evaluation, documentation, and follow-up.
• Maintain accurate wellness documentation and ensure compliance with Thrive policies and regulatory standards.
• Communicate effectively with residents, families, and healthcare providers to ensure collaborative care planning.
• Support clinical compliance and infection control protocols, ensuring safety and wellness in every aspect of care delivery.
• Promote a resident-centered care model, quality care outcomes, and continuous improvement through active participation in clinical meetings and quality assurance initiatives.
• Assist team members with clinical support and lead by example in professionalism, compassion, and skill.
What You Bring - Your Skills and Heart
• Current LPN/LVN license that is in good standing with BON.
• Graduate of an accredited nursing program.
• At least 1 year of nursing experience in home health, assisted living, hospital, or long-term care preferred.
• Strong clinical judgment, attention to detail, and the ability to manage changing priorities.
• Passionate about working with older adults and creating a joyful, wellness-focused environment.
• Proficient in Microsoft Office and EMR systems (or willing to learn Thrive's applications).
• Able to work flexible hours, including evenings and weekends, to ensure high-quality resident support when it matters most.
