The Long Term Care - LPN provides basic medical care, including changing bandages, administering medication, and collecting specimens. Administers nursing care in a long-term care environment under the supervision of a registered nurse or other medical supervisor. Being a Long Term Care - LPN monitors and reports changes in patient's condition to supervisor. Ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. In addition, Long Term Care - LPN requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Long Term Care - LPN's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.

Responsibilities:
- Directs and assists with resident, family/significant other education and notifies family on condition changes;
- Updates plan of care when needed and observes and documents observations and care given to the residents;
- Assists geriatric patients in Long Term Care (LTC);
- Collect samples for lab testing as assigned;
- Assists with formulating a plan of care for residents and provides nursing care to residents according to physician orders and regulations;
- Provide emotional and psychological support when needed.
