Job Description

Job Description
We are seeking an experienced Long-Term Care LPN for a traveler assignment. The LPN will provide compassionate, high-quality nursing care to residents in our Long-Term Care unit. Responsibilities include medication administration, assessments, care planning, documentation, and coordination with physicians and the interdisciplinary care team.
Responsibilities
- Provide high-quality nursing care to residents in a Long-Term Care setting.
- Administer medications and perform clinical assessments.
- Develop and implement individualized care plans.
- Document patient care and coordinate with healthcare team members.
- Support residents with daily activities as required.
- Active Hawai'i LPN license.
- Current BLS certification.
- Minimum of one year of recent Long-Term Care or skilled nursing experience.
- Experience with electronic medical records.
- Ability to work independently in a fast-paced environment.
Must be available for all shifts, including weekends and holidays as needed.
