Responsibilities:
- Provide direct patient care including wound care, medication administration, and vital sign monitoring
- Develop and follow individualized patient care plans in coordination with RNs and physicians
- Perform skilled nursing assessments and document patient progress accurately
- Educate patients and family members on care procedures, medications, and health management
- Communicate changes in patient condition to supervising RN or physician promptly
- Maintain accurate and timely clinical documentation in compliance with home care regulations
- Collaborate with interdisciplinary care teams to ensure continuity and quality of care
Requirements:
- Active Florida LPN license in good standing
- Previous home care, long-term care, or clinical nursing experience preferred
- Strong knowledge of medication administration and wound care protocols
- Excellent communication, documentation, and organizational skills
- Ability to work independently with minimal supervision in a home care setting
- Valid driver's license and reliable transportation required
- CPR/BLS certification required or ability to obtain upon hire
- Compassionate, patient-focused demeanor with a commitment to quality care
