Key responsibilities Take assigned cases and build execution plans based on urgency and patient needs Document interactions and support activities accurately to facilitate seamless handoffs Maintain communication with patients to update them on progress and gather necessary information Required qualifications An active LPN/LVN license or Community Health Worker (CHW) certification, or equivalent experience 2+ years of patient- or community-facing support experience Working knowledge of the U.S. healthcare system and insurance Ability to manage multiple urgent cases independently while maintaining strong documentation Reliable computer, internet, and a quiet space for patient calls
