Licensed Practical Nurse (LPN) - Rehabilitation and Nursing Center
Location: Mohawk Valley, NY
Assignment Duration: This is a 13-week contract position.
Schedule: Shift: 6:30 AM - 2:30 PM | EOW required | Floating required within the same building
Job Description: We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in a Long-Term Care (LTC) facility. The LPN will be responsible for providing quality nursing care to residents, collaborating with the healthcare team to support patient needs.
Responsibilities:
- Provide nursing care to residents in accordance with their individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document resident's health status and report any changes.
- Assist with daily living activities and promote resident independence.
- Participate in interdisciplinary team meetings and care plan updates.
Required Experience / Certifications / Licensure:
- Must possess an active LPN license in New York.
- Minimum of 1 year of experience in a Long-Term Care facility.
- Current Basic Life Support (BLS) certification.
- Completed Skills Checklist matching the specialty.
- Drivers license with address matching demographics.
- NURSYS Report for license verification with all active licenses.
