Position Highlights
* Department: Home Care Med Surg
* Hours: 40.00 per week
* Shift: Shift 1 - Days
Position Summary
Applies the nursing process with patients and families, integrating principles that reflect sensitivity to their age-specific/developmental needs, gender, culture, religion, psychosocial and socioeconomic backgrounds in order to provide evidence-based, holistic care in a manner that respects the worth and dignity of each individual. Practices professional nursing as a licensed practical nurse within the legal and ethical framework established by the Connecticut Nurse Practice Act and the American Nurses Association Scope and Standards for Nursing Practice and Code for Nurses, American Nurses Association Home Health Nursing, National Hospice Association and applicable regulatory agencies.
Essential Duties & Responsibilities
* Contributes to the nursing assessment by: collecting, reporting and recording objective and subjective data in an accurate and timely manner
* Data collection may include: observation about the condition or change in condition of patient, signs and symptoms of deviation from normal health status
* Participate in the development of the strategy of care in consultation with the RN by providing data, contributing to the identification of priorities and contributing to setting realistic and measurable goals
* Participates in assisting and providing direct care to patients
* Implement nursing care according to the priority of needs and goals established by the RN
* Provides an environment conducive to health and safety
Minimum Qualifications
* Graduate of an accredited Licensed Practical Nursing program
* Currently licensed as a Licensed Practical Nurse in the State of Connecticut.
* Has the knowledge and skills necessary to assess health needs of patients and to perform nursing care in a safe, caring manner.
* Willingness to obtain BLS certification within six months of hire
Preferred Qualifications
* One year of experience in homecare
* One year of experience working with electronic health records
* Active BLS certification
