Additional days or hours may be required.nn Travel required:nn No Travelnnn Job Summary: Professional supervision and direction is provided by the facility physician and/or Registered Nurse as assigned. Daily scheduling and direction is provided by the Facility Director. The LPN provides support to the physician in scheduling, assessments, obtaining orders and medication, maintaining documentation. The LPN passes medication and maintains accurate records of all professional activities. The LPN serves as resource for staff and residents and works a regular schedule and on call. nn Duties and Responsibilities: n Physician Authorizationsn Responsible for obtaining physician signatures as required on all documents.n Pass medications as ordered by the physician.n n Medical Administration Recordn Prepare MAR cards for use n Review and certify MAR cards are completed properly.n Keep track of all med orders and any new orders on the MAR cards.n Update MAR cards on all new medication orders and remove old orders from the card.n Audit MAR cards for compliance with orders.n Check for any missed medications.n Follow up with any missed meds with the appropriate staff.n Organize, schedule and document clients medical assessments,n As identified by the Physician obtain medication, schedule procedures, lab work etc. and follow up to insure clients needs are met.n Review medications with residents and ensure they are aware of possible side effects.n Collect health care reports, physicians orders, document future appointments and enter the information and track it on the clients discharge summary.n Make note of any medication changes or treatments in the Communications Log.n Monitor, document and follow-up on specialist or emergency room visits.n Maintain a regular on call schedule.n Work a flexible schedule as determined by the needs of the clients and availability of the physician.n Employees are required to use their personal vehicles in certain circumstances and as directed in the performance of their job duties.n Perform other duties as assignednn Required Skills/Abilities: n Excellent verbal and written communication skills.Knowledge of techniques used in de-escalationExcellent attention to detail in completing work tasksn Ability to be a team player and work with diverse populations. n Ability to lead, train, coach, and develop othersn Excellent instructional skillsn Ability to create curricula based on program needs nn Education and Experience: n Requires graduation from an accredited Licensed Practical Nurse program; Prefers 1-2 years LPN experience. Valid West Virginia LPN License; must have a pharmacology certificate.
