To support a remote healthcare team, the full-time LPN Case Manager Assistant will assist the RN Case Manager in case management functions, including service authorization, treatment plan reviews, and ongoing communication with healthcare team members. Key responsibilities Assist in case management functions under the supervision of the RN Case Manager, excluding initial assessments and care plan formulation Review medical data and communicate with providers and payers to update case management plans and verify medical necessity Document case actions and maintain billing in the computer system, ensuring timely reports and ongoing patient care coordination Required qualifications Current and unrestricted Licensed Practical Nurse (LPN) license or Medical Assistant/Nursing Assistant certification Preferred completion of Licensed Practical Nursing education Five years of varied clinical experience or three years in a specialty field Basic computer skills for documentation and case management Familiarity with community resources and funding sources for patient assistance
