LPN Care Coordinator – San Francisco, CA
TAG MedStaffing is hiring an LPN Care Coordinator in San Francisco, CA for our client. This on-site role supports chronic care management through care coordination, patient outreach, documentation, and engagement activities. You will use EMR and digital health platforms to maintain CMS‑compliant care plans while collaborating with RNs, neurologists, caregivers, and external providers. The role may transition to remote or hybrid work depending on client needs and requires candidates to reside in the San Francisco Bay Area.
Responsibilities
- Provide ongoing care coordination, follow‑up, and support for patients in chronic care management programs.
- Conduct virtual and phone check‑ins to monitor patient status, reinforce care plans, and encourage adherence.
- Assist RNs with updating care plans and documenting interventions in the EMR.
- Coordinate with physicians, caregivers, and clinical teams to support timely updates and continuity of care.
- Manage scheduling, documentation, data entry, and escalate clinical concerns to supervising RNs when needed.
Qualifications
- Active, unrestricted California LPN license required (multi‑state licensure preferred).
- Prior experience in chronic care management, care coordination, or telehealth, particularly working with older adult or neurodegenerative patient populations.
- Proficient with EMRs, digital health platforms, telehealth tools, and remote documentation workflows.
- Understanding of CMS PCM/CCM program requirements, quality improvement practices, and interdisciplinary communication standards.
- Empathetic, organized, and proactive communicator with strong problem‑solving skills and the ability to manage a hybrid patient caseload effectively.
